Policies & Exclusions

Eligibility

​

  • Membership is only good at Erin McLendon DDS & Associates

  • This is a membership it is not dental insurance.

  • Eligible dependents include your domestic partner and children under age 26.

  • Membership can not be combined with any other offers.​

  • If the patient elects to use dental insurance, insurance plan fees, payments, and deductibles will apply.

  • All patients are subject to Erin McLendon DDS & Associates office policies.

Exclusions

​

  • Membership fees and terms are subject to change yearly.

  • Membership discounts will not be offered for services requiring referral to a specialist. Referral to a specialist is at the discretion of the doctor.

  • Should treatment be needed following an injury or 3rd party outside insurance is involved, this membership can not be used.

  • Treatment initiated prior to membership is not eligible for a discount.

  • Prosthesis delivered or in-process treatment completed more than 60 days after termination of coverage is not eligible for membership discount.

  • Treatment fees are guaranteed for 3 months from the date quoted by the office.

  • Erin McLendon DDS & Associates reserves the right to discontinue membership for any patient at anytime, if the patient is dismissed from the practice.

  • No showing or cancelling of an appointment without 24 hour notice without a valid excuse will void the membership discount for the scheduled treatment.

Payments

​

  • All payments are due at the time of service to receive membership discount.  Any services not paid in full at the time of service will be billed at our regular fees.

  • Enrollment fees must be paid in full to receive discounts.

  • All payments are nonrefundable.

  • No refunds will be given if a member and/or domestic partner or children do not use the plan, relocate, or obtain dental insurance.

  • 12-month term effective from the date of application payment.
